  • GP details (if it's necessary to contact your GP we will have a discussion with you before doing so)
  • Your medical history that you share with us
  • Lifestyle habits, routines and preferences
  • The reason that you have requested therapy
  • Updates regarding your progress

What do we do with the information?

  • Contact you to arrange an initial telephone appointment and also send details of appointments. You will also be sent follow up emails during the therapeutic process
  • Name and email address will be passed on to the centre where the therapy sessions are taking place
  • Your email address will be used to share an electronic folder with you to enable you to access your personalised therapeutic recordings
  • Use the medical and lifestyle information to confirm suitability for hypnotherapy treatment, and to create a treatment plan for you. Updates will be used during your therapy to plan and guide future treatment

How long we keep your data

  • Client records will be kept securely for three years after your last appointment

Sharing your personal information

  • We do not share personal medical history of clients with any one unless you have expressly asked us to or there is a legal requirement to do so
  • During professional hypnotherapy supervision groups or one to one supervision some annonomised details may be shared  for professional support and development

Your rights

You have the right to ask us not to process your personal data for marketing purposes. We will inform you (before collecting your data) if we intend to use your data for such purposes or if we intend to disclose your information to any third party for such purposes.
